Many people flinch at the sight of a large spider. But what does it feel like when this fear becomes so overwhelming that it interferes with everyday life?

When Fear Becomes a Phobia

In this case, we speak of a specific phobia. It is defined as a pronounced, excessive fear of a particular object or situation that leads to avoidance and significant distress in everyday life (see ICD-11).

The Concept of Preparedness

Why do we react so strongly to animals such as spiders, even though an ordinary house spider cannot harm us? One explanation is the concept of “preparedness” (Seligman, 1971). According to this theory, humans are evolutionarily prepared to associate certain stimuli with fear particularly quickly—namely those that posed a real threat to our ancestors’ survival.

Those who immediately fled had a survival advantage. The body activated its “fight-or-flight” response so that we could quickly escape danger. This response is deeply rooted in us and is still triggered today when we see a small house spider. This explains why some fears are so persistent and can hardly be switched off simply by knowing that there is no danger.

The Role of Exposure Therapy

This is precisely where exposure therapy comes in. People with a phobia usually avoid the feared stimulus. This brings short-term relief, but maintains the fear in the long term. They never get the chance to experience that the feared catastrophe does not occur. In exposure therapy, people therefore learn to face the feared situation in a controlled way until the fear response subsides and new, corrective experiences become possible.

Virtual Reality Exposure Therapy (VRET)

Virtual reality exposure therapy moves this confrontation into a virtual environment. Patients immerse themselves in a situation that realistically depicts the feared stimulus, such as a spider moving around the room. The portrayal is convincing enough to actually trigger the fear response. This is crucial, because fear can only be addressed in therapy when it becomes perceptible. At the same time, patients know that they are in a protected setting.

Compared with traditional exposure in the real world, VRET offers several practical advantages. The intensity can be precisely controlled, exercises can be repeated as often as needed under the same conditions, and the confrontation takes place in a protected environment. It also lowers the barrier to getting started. For many people, even imagining facing the real trigger is difficult. The virtual version can make it easier to begin treatment.

Virtual reality therefore expands the ways in which exposure can be delivered and makes it accessible to more people.

Seligman, M.
